Note: If you're not using the same forum ACCOUNT that you had as an admin & your Minecraft name and Twitter handle are not the same as when you we're a past admin, you will have to verify your identity with another admin that can vouch for your identity.
3. What rank were you (Admin,Moderator,T-Mod, etc.) and what date did you get removed (e.g. July 24th 2014)
4. Were you removed from admin for misconduct or reasons other than inactivity? If yes, give full explanation
5. Why do you wish to be reinstated?
6. If you are reinstated, how often will you be able to do admin duties?
7. Are you completely familiar with the server rules and Commands to be reinstained
